The Arcadia Public Library is an authorized acceptance facility designated by the U.S. Department of State to process DS-11 Passport Applications.

DS-11 Applications Accepted For:

  • First-time passport applicants
  • Adults whose passports have expired and who are not eligible to renew using the DS-82 application
  • Minors whose passports are expiring or have expired

DS-11 application forms are available at the Library or may be obtained from the U.S. Department of State website at travel.state.gov.

DS-82 Renewal Applications

The Arcadia Public Library does not process DS-82 Passport Renewal Applications.
Applicants who are eligible to renew must submit the DS-82 application directly to the U.S. Department of State by mail or renew online.

Required Documents

To ensure your appointment proceeds efficiently, all applicants must be present and must bring the following documents:

  • Proof of U.S. Citizenship
    • Accepted documents include a certified Birth Certificate (abstracts are not accepted) or Naturalization Certificate.
  • Proof of Identity
    • A valid California Driver’s License, current passport, or government-issued photo identification.
    • Applicants presenting a state-issued identification or out-of-state Driver’s License must also provide a secondary photo identification.
  • Passport Photo
    • Passport Photo services are available onsite for $16.
  • Minors 15 years and under
    • Both parents must be present with the minor applicant.
    • If one parent is unavailable, a notarized statement of consent must be submitted along with a photocopy of front and back of their Driver’s License.
    • Parents presenting an out-of-state Driver’s License must provide a secondary form of photo identification.
  • Minors 16 or 17 years
    • Must be accompanied by one parent when applying.

Emergency Travel

Applicants traveling within two (2) weeks must schedule an appointment directly with a U.S. Passport Agency by calling (877) 487-2778. Proof of travel is required.

First-Time Passport Book $130 payable to the U.S. Department of State (Check or Money Order ONLY)
First-Time Passport Card $30 payable to the U.S. Department of State (Check or Money Order ONLY)
First-Time Passport Book and Card $160 payable to the U.S. Department of State (Check or Money Order ONLY)
Processing Fee $35 payable to the City of Arcadia (Cash, Check or Money Order, Visa or MasterCard)

MINOR (15 years & Under) – Passport Book, Passport Card
Passport Book $100 payable to the U.S. Department of State (Check or Money Order ONLY)
Passport Card $15 payable to the U.S. Department of State (Check or Money Order ONLY)
Passport Book and Card $115 payable to the U.S. Department of State (Check or Money Order ONLY)
Processing Fee $35 payable to the City of Arcadia (Cash, Check or Money Order, Visa or MasterCard)

Expedite Fee (allow 3-5 Weeks) $60.00 payable to the U.S. Department of State (Check or Money Order ONLY)
1-2 Day Delivery (passport mailed to customer from U.S. Department of State)  $21.36 payable to the U.S. Department of State (Check or Money Order ONLY)

Fraud Warning

The Arcadia Public Library does not process passport applications online.
Applicants are advised to remain vigilant against online passport fraud. To report suspected fraud, please visit the U.S. Department of State website at travel.state.gov.
