The Fair Political Practices Commission (FPPC) requires campaigns to file various disclosures regarding campaign contributions and expenditures.

Meaning of Common FPPC Forms  

FPPC Form 401 - Slate Mailer Organization Campaign Statement

This form is used to disclose payments made and received in connection with producing slate mailers. A "slate mailer" is a mass mailing that supports or opposes a total of four or more candidates or ballot measures.

FPPC Form 410 - Statement of Organization Recipient Committee

A group or entity qualifies as a recipient committee when contributions of $2,000 or more are received. A recipient committee is required to file a Statement of Organization (Form 410), which provides the public with information about the purpose of the committee and identifies the committee’s treasurer and principal officer(s). 

FPPC Form 460 - Recipient Committee Campaign Statement

Typically, all general purpose committees (state and local) use the Recipient Committee Campaign Statement (Form 460) to report campaign activity for all semi-annual and pre-election filings. The Form 460 is the main campaign disclosure report and provides the public with an overview of the committee’s activity, including money coming in and money going out, during a specified reporting period. 

FPPC Form 470 - Officeholder and Candidate Campaign Statement  

Form 470 is for use by officeholders and candidates who do not have a controlled committee; do not anticipate receiving contributions totaling

$2,000 or more during the calendar year; and do not anticipate spending $2,000 or more during the calendar year. Officeholders and candidates who have a controlled committee or who have raised or spent $2,000, file the Recipient Committee Statement – Form 460.

FPPC Form 501 - Candidate Intention Statement

A candidate for state or local office must file this form for each election, including reelection to the same office.
